|Finding List (Directory) Form - Due June 15|Required for all new students. For returning students, needed for changes in address, phone, parent-e-mail,
etc. This form was sent by mail to all new and returning families. If changes are not received, your listing in the Finding List (school directory) and in the online Directory will be the same as 2012-13. Please return forms to school offices by June 15. |Health Forms Grades 9-12 - 5 Forms Due June 13|
The five forms in this packet are required to be completed by all students enrolled for the 2014-2015 school year. All forms are due by June 13, 2014.
1. Emergency Contact, Waiver, and Consent Form • Must be completed and signed by a parent
2. History Form • Must be completed and signed by both the student and a parent.
3. Physical Examination Form • To be completed by a physician • Attach a current/updated Immunization Record
4. Medication Release Form • Must be signed by a physician if there are prescription medications and/or procedures required to be taken or carried out during school or on school related trips. Must also be signed by a parent. • If the above does not apply, must be signed by a parent only.
5. Concussion Release Form - Must be signed by both student and parent

|Bus Transportation Application - Due July 15|
Download the form at right for information and the application for Bus Transportation. Return the form to the Business Office at the High School campus (mail to 7401 Highway 100, Nashville TN 37221) by July 15.
|School Dress (Uniform) Information|
Download the EHS Uniform Brochure at right for information about requirements and options. (Note that the brochure has four pages in the download.) Additional information about June sales at Parker Uniforms is available in the downloads at right.
|Calculators for Math Courses|
Students in Algebra 1, Integrated 1, Integrated 1 Honors, Integrated 2, and Integrated 2 Honors will be using the TI-Nspire CX graphing calculators in 2014-15. Families can purchase one from the school for approximately $125. Please place your order soon by sending in a paper copy of the form to Sharon Glenn. Calculators will be distributed during the first few weeks of school, and they can be engraved with the student's name. Download the Order Form at the right on this page.
Students in all other math courses will use the same TI 83 and TI 84 models as they have used in the past.

. You'll find a general information letter to parents, an overview for textbook purchasing for the Follett Books Virtual Bookstore. The list of textbooks for the 2014-15 year is found as a download at the right on this page.
Parents will receive information about your student's courses for the 2014-15 year in early July after the HS course scheduling process has been completed. Please note that before that point, we will not know in which semester particular courses will run or even whether scheduling conflicts might cause your child to use one or more of the alternates that they identified for their first choices. The Follett Virtual Bookstore will open July 7, 2014. This resource is provided as a convenience for parents; other sources may be used if preferred.
|Introduction to Macbook Course|
Every student carries a MacBook laptopwhile attending Ensworth’s High School. As a result, technology is deeply woven into thecurriculum. This course equips each student with the knowledge and skills to be responsible and productive with this exciting, educational tool. Through engaging, hands-on exercises and projects students are given instruction ona variety of applications used daily by every student at the High School. Upon completion of this one-day course students are allowed to take their laptops home. Note: Students attending the Introduction to
Ensworth Course do not need to take this course.

|Laptop Computer Information|
For 2014-15, rising 9th graders and new students in grades 10-12 and their parents will need to complete the Laptop Acknowledgement Form available as a download at the right on this page. The forms are due by the start of school (orientation) in August.The "Ensworth Laptop Acknowledgement Form 2014-15" is the only document that must be signed and returned; the other two documents are informational and do not need to be returned.
